We are excited to welcome you as a new patient!

Please fill out our new patient forms prior to your first visit. Getting this done in advance allows you to have more one-on-one time with the doctor at your appointment. You can print and bring the forms with you or send them to us in advance by email or fax.

Your first visit

Consultation

On your first visit you will have a consultation with one of our doctors to discuss any health-related problems or concerns you may have. This allows them to learn more about you and your health, and gives them an opportunity to determine how chiropractic care can meet your needs and get you moving toward your personal goals.

Examination

After your consultation, the doctor will perform a complete chiropractic examination including a full spinal assessment, muscle testing and any other procedures that are necessary such as x-rays.

Your Second Visit

Personalized Care Plan

After your first visit, your doctor will create a personal care plan for you. Care plans vary from person to person in frequency, recommendations and length of time. Healing is always a process. Your care plan will depend upon many factors, including how much stress you have had in the past, how much stress is on the nerve system currently and your lifestyle habits. Your personal care plan is designed to meet your health needs as well as your health and personal goals.
